New Patient Info
To expedite the process of new patient entry in our systems, we invite you to download, print and fill out the forms below. Please bring them with you on the first visit, in addition to your medical insurance card.

Insurance
We are in network with most major insurance providers. If your medical insurance information, contact/billing information, e-mail address or any other particulars have changed, please fill out the form below so we can update our records.

After Hours Care
If you need after hour care, please call the main line at 770.287.1140 and follow the prompts.  After you leave a voicemail, and a doctor will be paged who will respond to your call. Normal hours are: Monday – Friday: 9am to 6pm.

Patient Privacy
At Oakwood Family Physicians, we take pride in educating our staff about privacy guide lines outlined under HIPPA (The Health Insurance Portability and Accountability Act).  We recognize the importance of your privacy and will not sell personal information to third parties for commercial use. All privileged information between patient and physician is handled in the most professional and secure manner.
